When I first got into the fire service, my first CPR “save” was a 42 year old guy who had a heart attack on the golf course, down 10 mins no CPR, we got a pulse back, transported and celebrated the save, 2 months later we ran on him at a nursing home, feeding tube, 100 percent dependent on care, the time down really matters
  5. Not a doctor, similar background as Herbie, all these things pretty much follow a script and it all depends on how long he was down without 02 and if his heart suffered any permanent damage.

They’re weaning him off the ventilator, to see how he breathes on his own, pretty normal after a cardiac arrest event, brain activity can be measured through different tests, severe damage can be seen on a CT, and brain activity measured on an eeg, anything in between its pretty much wake up and assess from there, the person who said earlier that oxygen is a drug is 100 percent correct, they won’t keep him on a vent longer than required because you can absolutely become dependent, someone already has a pretty good idea of what’s going on and they’re keeping it private as they should.

I think a lot of that was probably third hand, I cannot fathom any waiting for a patient who is pulseless or even regained ROSC, the goal of paramedics is always definitive care and that’s a hospital every time. Now a doctor could of assumed care and stalled transport but if so he’s gonna have some questions to answer and a lot of liability.
  8. If his pulse is back it’s great and indicates discontinuation of CPR, it’s how long it was stopped and his brain without 02 that comes into play now.

  10. Speculation only, but if it was commotio cordis, it’s very survivable because it’s cardiac arrest without an underlying condition secondary to impact. Defibrillation(shocked back into a normal rhythm that allows the heart to create a pulse) is the only treatment, and it would need to be rapid, the 9 min down time if he was pulseless is an issue to long term brain function.

  12. It’s more than likely not aneurysm, standing up like that and then falling over immediately is a rapid, massive drop in blood pressure
  13. Get hit precisely at the right place at the wrong time at the peak of the T wave can cause ventricular fibrillation.
  14. Commotio Cordis, or a massive aneurysm. Hopefully he wasn’t pulseless for 9 min before cpr began.
