  3. You’re making a big leap that single payer means that payments automatically go down to the floor currently set by CMS. Moving to single payer doesn’t mean that all current reimbursement methodologies are adopted. You’re spouting the insurance lobby talking points without putting any thought into how we might redirect dollars already in the healthcare system.
  4. So much this. Insurance companies and pharma are primarily responsible for the high cost of healthcare in this country. Physician spend is a pittance in comparison.

  6. This can no longer happen. The No Surprises Act prohibits balance billing by facility based providers. That said, as is mostly always the case, there have been unintended consequences in how the law has been implemented that has given insurance companies additional leverage over providers. So the legislation is good for patients, good for insurance companies and not so great for physicians.
  7. This impacts the anesthesiologist, not the hospital since it’s addressed to professional fees. If the anesthesiologist is contracted with the health plan, they will likely not be able to pass the cost on to the patient. The only leverage the anesthesiologist has in this case is to term their contract with the health plan and attempt to renegotiate. Now that the No Surprises Act (good idea in concept, not as much so in execution) is law of the land, the insurance company knows that the anesthesiologist won’t be able to bill the patient for any difference between what they decide to arbitrarily pay out of network and the anesthesiologist‘s fee so they hold the doc by the balls unless the anesthesiologist wants to take the health plan to arbitration on every single claim they submit.

  9. Fixed for clarification. BCBS is a social welfare company. Elevance, formerly Anthem, is a company that started buying up individual Blues in the late 90’s and later went public. They are now the largest owner of Blues and is squarely in the for profit space which, when first started, was not something envisioned by BCBSA.

  20. Boise is considered semi-arid. I love the weather there. Four distinct seasons and winters are pleasant with some snow and cold weather, but nothing completely miserable. They get about 1/3 of the annual snowfall that Denver does. If you think you might be interested in Billings, I would add Bozeman to the list. Billings is pretty meh in my book, although it has gotten a little better over the past couple of years. Billing is more industrial/blue collar while Bozeman is much more scenic and has the amenities of a university town.
