There are two competing strains of thought circulating right now, and try few people seem to keep both points in mind simultaneously:

1. The current data almost certainly overestimate the CFR to date, due to severity bias.

2. The current data likely understate the CFR we will see as the virus spreads more widely.

Both are true. It’s possible to acknowledge the former while designing policy with the latter in mind.

That’s the one. I wasn’t at that Wednesday service so whatever I have is probably just normal stuff. But my symptoms are consistent . . . .

Slowing the spread to save the healthcare system has been stated over and over and over as the #1 goal right now and asymptomatic spreaders are the #1 problem that will prevent that goal.

It never really got contained, it just burned through the global population and mutated to a far less deadly strain.

One week ago I was pretty sanguine. I am a total doomer, witness my being all over page 1. So for six weeks I watched closely, even bought extra food in the pantry, but wasn't really freaked. My brother in law lives in HK, and so he had a closer perspective. Containment is working there. I'm youngish, my children are hale. The cases in China, SK, and Japan seemed to be leveling off so much. We've been at or under 100k for a long time - steady climbing but it didn't seem out of control. I didn't really believe China's numbers. But there were fewer and fewer exported cases from China - lending some credence to the possibly that China proved it could be contained. Neighboring states seemed to be weathering pretty well. The SK cases were a unique blow up, but even that showed major flare ups could be dealt with.  Iran is a shitshow, but I chalked that up to bad government and the impact of years and years of embargos on the health care system. 

But then Italy and Washington State (and Egypt)... and probably more scare are all the cases those two hotbeds have exported cases. As someone mentioned this AM, there is a case that Las Vegas exported, and we have no idea what things are like there. Now we have SK + Iran + Italy + Wash St + Egypt... probably other clusters across Europe, plus Indonesia and maybe India... just so many more chances to break  out.  

So this weekend, I feel far far worse about things. I do think any given regional blow up can probably be successfully curbed. It might take months, but there is hope. But if there are dozens of regional flare ups? 

Prediction: Be ready for schools to extend Spring Break 2-3 weeks.

Even if you believe in small, limited government I would think coordinating a plan to deal with a global pandemic would fall under the federal government's duties.

It used to be the Churches in coordination with Christian doctors.  Hence the names.... Presby, St. Enter name here, Baylor Scott....., Methodist...  FDR took over in the 30's.  This is when hospitals got expensive.  Used to be charity work.  In other words, free.  Well, medicaid is still technically free, the people just don't pay.

“I'm a doctor and an Infectious Diseases Specialist. I've been at this for more than 20 years seeing sick patients on a daily basis. I have worked in inner city hospitals and in the poorest slums of Africa. HIV-AIDS, Hepatitis,TB, SARS, Measles, Shingles, Whooping cough, Diphtheria...there is little I haven't been exposed to in my profession. And with notable exception of SARS, very little has left me feeling vulnerable, overwhelmed or downright scared. 

I am not scared of Covid-19. I am concerned about the implications of a novel infectious agent that has spread the world over and continues to find new footholds in different soil.  I am rightly concerned for the welfare of those who are elderly, in frail health or disenfranchised who stand to suffer mostly, and disproportionately, at the hands of this new scourge. But I am not scared of Covid-19.

What I am scared about is the loss of reason and wave of fear that has induced the masses of society into a spellbinding spiral of panic, stockpiling obscene quantities of anything that could fill a bomb shelter adequately in a post-apocalyptic world. I am scared of the N95 masks that are stolen from hospitals and urgent care clinics where they are actually needed for front line healthcare providers and instead are being donned in airports, malls, and coffee lounges, perpetuating even more fear and suspicion of others. I am scared that our hospitals will be overwhelmed with anyone who thinks they " probably don't have it but may as well get checked out no matter what because you just never know..." and those with heart failure, emphysema, pneumonia and  strokes will pay the price for overfilled ER waiting rooms with only so many doctors and nurses to assess. 

I am scared that travel restrictions will become so far reaching that weddings will be canceled, graduations missed and family reunions will not materialize. And well, even that big party called the Olympic Games...that could be kyboshed too. Can you even 
imagine?

I'm scared those same epidemic fears will limit trade, harm partnerships in multiple sectors, business and otherwise and ultimately culminate in a global recession. 

But mostly, I'm scared about what message we are telling our kids when faced with a threat. Instead of reason, rationality, openmindedness and altruism, we are telling them to panic, be fearful, suspicious, reactionary and self-interested.

Covid-19 is nowhere near over. It will be coming to a city, a hospital, a friend, even a family member near you at some point. Expect it.  Stop waiting to be surprised further. The fact is the virus itself will not likely do much harm when it arrives. But our own behaviors and "fight for yourself above all else" attitude could prove disastrous. 

I implore you all. Temper fear with reason, panic with patience and uncertainty with education. We have an opportunity to learn a great deal about health hygiene and limiting the spread of innumerable transmissible diseases in our society. Let's meet this challenge together in the best spirit of compassion for others, patience, and above all, an unfailing effort to seek truth, facts and knowledge as opposed to conjecture, speculation and catastrophizing.

Facts not fear. Clean hands. Open hearts.
Our children will thank us for it.

I’m in Vegas...far from panic mode here.  I don’t think anyone should panic.  It’s a physician convention here and not one doc seems panicked - I’d guess the ones that are panicky stayed home, but I digress...  Not panicking and not testing are two different things.  We should absolutely be testing more.  Pretty much everyone here agrees on that.

I had this conversation with an uncle last night.  He did the “it’s only 2% and the President said it’s not even that” and I pointed out that for his age range, it’s around 10%, and because he has other “underlying” issues, that’s probably added to it. He did quit smoking and a few other bad habits nearly 15 years ago after having to be hospitalized for a stroke just after his first great-grandkid was born, but I would be surprised if wasn’t pushing 20%.   His wife had cancer 5 years ago, and she’s got the beetus, so she’s probably up there.   

It really is easy for many to be flippant about it, and blow it off, but if you’re older, or have older relatives, it’s best to start looking at age groups and why the people with underlying issues seem to be the ones being wiped out.

I'm definitely a superspreader.  When I'm sick, I pop and advil and head out to work or whatever....  Exposure to my wife for extended periods is fatal.
