There's a legitimate argument to be made that the shooter just took out a serial killer.  If you're doing everything in your power to deny paying out on legitimate healthcare claims which causes tens of thousands of deaths that could have otherwise been averted, what else can we call you?  You can't just blame the system.  There can't be zero accountability.  These folks had a faulty AI that they knew was faulty and they kept it because it was denying legit claims and making them more money.  That's grim as fuck.

How about we start with the taxpayers that fund the vast majority of the foundational and a good portion of the translational research that then gets monetized by Pharma? That $20k prostate cancer drug, yeah that doesn't exist without the US taxpayer. But we don't get to realize any return on that investment. We do get, otoh, to pay higher premiums as result of the risk share. But sure, UCLA gets a piece. WTF? Totally nonsensical shit. We need to fundamentally turn the whole system over. Can't just fuck around with one component.

Not aimed directly at you, sorry. Just making this clear for non industry folks.

Let's not act like insurance companies are even anywhere close to the same boat as providers, hospitals, DMEs, EMR companies, transport companies, etc when it comes to the cost of healthcare, because they absolutely are not.

UHC made 32 billion last year in profit. Elevance and Cigna were over 7. Aetna was at 13.7. Humana was at 4. Cetene was at almost 3. 

For comparison, HCA as an entire company was at 5 and change for 2023. Tenet was at 1 and change. UHS was at 717 million. CHS lost money. Encompass was under a billion. Ascension lost 2.66 billion.

They aren't the same.

I had to wait 4 months to see a neurologist a couple of years ago, and the only reason I got in that quickly is because his daughter was one of my favorite former students and I contacted her and asked if she could help me get her dad's attention.

If every doctor worked for free in the US, you would save a total of 15% of the US healthcare expenditure. This is part of the frustration. Front and center as the face of the problem, and not even close to being true.

As one of the owners of a small/medium sized business, which provides health insurance for its employees, maybe we bosses should be shot.  But from our perspective, we do the best we can with the system that we have.

1) If our employees are to have healthcare worth a shit, it's going to be with an employer-provided plan. So, we HAVE to be in the health insurance game, whether we want to be or not.

2) We actually want a plan that offers good coverage, minimal hassles, and leads to good outcomes.  ALL of us are on the plan (owners and employees), so we all have an interest in it providing good outcomes.  That, and (not to put a halo on our head or anything) we actually want our employees and their families to be happy and healthy.

3) Sure, cost is a factor for us.  We employ a broker who shops around for us.  We've even changed insurance carrier before (we switched over to another, were there for three years, then switched back, preferring our original carrier).

So, maybe some of it depends on the moral compass of management at your employer.  But even if it's not a moral compass, there's an objective good policy in having your employees and their families be healthy, and be treated promptly and well for any illness.  It keeps them from falling off the job, from being unduly distracted, etc.  So, even a self-interested employer should want their health plan to be a good one.  But, through all of that, we really can only select from a menu of choices that exist.  We can't really craft a whole new recipe on how to deliver healthcare.  Our choices are "do you want to go with carrier X, Y, or Z?"  And our decision is driven by how well that carrier performs, whether there is a good pool of in-network providers in the area, and yeah, to some extent, cost.  Opting out or creating something entirely different isn't an option.

TLDR: our system has placed the burden of setting up healthcare for people on the backs of employers.  I suspect that many, even most of them, do their best to select the best option from a menu of shitty choices.  After all, employers are not healthcare experts either.  Their core business is selling auto parts, or accounting, or manufacturing widgets, or whatnot.

Michael Davidson got shot by the son of one of his patients, because he thought he was at fault for a complication she sustained.

There are other examples. 

My patient population skews heavily towards the knife and gun club, and have had 2 bring firearms into the hospital. I know I've asked myself if it is worth managing such a highly volatile group.

We are already estimated to be facing physician shortages longterm. And when more people start opting out into concierge medicine alone, I don't think many of you are going to be happy with the results.

Just my perspective.

Yep.  Which is why we, as a society, should actually try to AVOID things that are utter systemic shitshows.  Because once you start blowing shit up, the shrapnel goes everywhere, much of it indiscriminately.

That's why the SMART tech bros/billionaires are thinking and saying "so....maybe massive wealth inequality, while leaving the masses to suffer shitty outcomes, is a recipe for unrest and destruction of the very thing (a functioning society) that helped us prosper in the first place."  

People who became wealthy because of our social and legal structure....being okay with the destruction of that social and legal structure....are killing the goose that laid the golden egg.  It's so fucking stupid, counter-productive, and shitty.  But....all gas, no brakes.  We've clearly made our choice -- fuck, the majority of our society (stupidly) cheers it on, they LOVE them some billionaires who OPENLY say "the people gotta suffer, that's just the way it is."  I wish we would choose otherwise, but that wish is never coming true.

What would happen if we took the 50+ billion in profits skimmed off the top by insurance companies (not to mention all the money payed out to employees in bonuses for refusing coverage effectively) and put that money back into the system? 

 

That's why I don't buy this argument. Take out as many for profit middle men in every industry that is essential to health and basic needs.

Charlie Work, but Charlie can't read so he ended up on the train to Brookyln instead. Mac was next on the list to do it, but thought the target was too good looking and fell in love instead and tried to stop Dennis along with Dee who thought it was a bad idea. Dennis had to do it himself in the end. Frank was hiding in the backpack for support.
