Spatial disorientation/CFIT with a non-instrument pilot has killed so many in aviation. That's what got Buddy Holly and Otis Redding and many others.

Wait...was there a question of whether or not a helicopter crash killed him? I'm pretty sure it was a helicopter crash.

yes and no. Kobe was killed in a similar accident, VMC into IMC (clear to clouds), with a disorientated, non-instrument rated pilot. This is extremely common in general aviation accidents. They say once a non-instrument pilot gets in IMC, you have about 90 seconds to live.

That was the presumption of what happened to SRV. But there wasn't much fog or IMC the night SRV died. Instead, the disorientation was caused by complete darkness with no visual references. So still spatial disorientation and CFIT, but slightly different. Fly commercial folks.

or make sure you only ever get instrument-rated pilots
